Social Services

Social Workers are visible in the hospital and available to all departments, hospital staff and physicians. Social workers are also available to all Wise Regional Health patients including Emergency Room, Outpatient Services and Home Health.

What We Do:

Discharge Planning - Planning and arranging post hospital care to consolidate gains made during hospitalization.

Crisis Intervention - Helping patients and families adjust to trauma and catastrophic illnesses especially during Code Blues and other traumatic/emotional situations.

Counseling - Enhancing patient/family functioning in times of illness utilizing problem solving processes.

Consultation - Providing specialized knowledge to physicians and other health care personnel regarding patient psychological problems and procedures or services available to the patient/family.

Information and Referral - Providing information to hospital staff, patients and families regarding types of community resources available Contacting organizations on behalf of patients and their families for psychiatric treatment, financial assistance, rehabilitation, home services, educational plan or other special needs.

Financial Evaluation - Securing information regarding employment or financial status for use by the physician or business office in determining eligibility for financial aid. Linking patients to resources if eligibility is determined.
